Due to unexpected and unforeseen circumstances, we must close our practice sooner than originally anticipated. Serving Summit County and the surrounding mountain communities for the past 13 years has been one of the greatest privileges of my life. Melissa and I have been incredibly blessed by your loyalty, support, and business over the years. I will deeply miss seeing you in the clinic, hearing your stories, and caring for your vision. Thank you for trusting me with your eye health. As this chapter closes, an exciting new one begins for our family. Melissa and I have joined LightBridge International and will be relocating to Cambodia to establish a non-profit eye clinic. It has long been a dream of ours to bring sustainable vision services to my native country, and your support over the last decade has helped make this mission possible. Your health and continuity of care is our priority, therefore, please review the following details regarding your medical charts and outstanding orders. Please note that I am no longer seeing patients for exams. If you require immediate optometric care, we encourage you to contact another local eye care provider in the area as soon as possible. If you have outstanding orders for eyeglasses or contact lenses, or if you would like to pick up a physical copy of your current prescription or medical records in person, you must do so before this date. Medical Records Effective 7/28/26 we have retained Cariend, a medical records custodian, to manage the records for the next 22 years. Cariend is not a medical provider and does not handle billing, medications, or referrals for the closed practice. Requestors should be prepared to complete the Records Release Authorization Form and provide a copy of a government-issued photo ID, as required by HIPAA.
